Vulnerable Person Registry
Definition

The Vulnerable Person Registry allows an improved police response to vulnerable people who may require emergency assistance due to their condition. It expedites the process of locating and assisting vulnerable residents or frequent visitors to York Region by making essential personal information readily available to responding officers.

 

Examples A vulnerable person is someone with a cognitive, physical, intellectual or developmental disability or other condition which may place them at an increased risk of misadventure. These conditions may include:
  • A tendency to wander
  • The inability to communicate
  • Fascinations or attractions to dangerous locations (water, construction sites etc.)
  • Unusual social responses (fear of strangers, aggressive nature)

York Regional Police will use the personal information provided to us to respond to calls for service and/or assistance, to assist with interacting with the persons named in this form, to administer the York Regional Police Vulnerable Person Registry program, and for other law enforcement purposes. The personal information provided to York Regional Police may be disclosed to other law enforcement agencies for a consistent purpose. The personal information provided to York Regional Police, York Region Paramedic Services, and Fire Services within York Region may be disclosed to other law enforcement agencies for a consistent purpose. It is your responsibility to ensure that the information submitted is current and valid and that York Regional Police are notified of any changes via the Vulnerable Person On-line Registry. The retention, as well as any other use or disclosure of personal information will be governed by the requirements of the MFIPPA and York Regional Police’s retention by-law/schedule.

I acknowledge that I am the Registrant or parent/guardian of the Registrant or have been legally designated to make decisions on behalf of the Registrant. I hereby consent to the collection, use, retention and disclosure of the personal information provided to York Regional Police.

In consideration of York Regional Police’s compliance with the collection, use, retention and disclosure of personal information as set out above, I hereby release, waive and forever discharge York Regional Police, the Chief of York Regional Police and the York Regional Police Services Board from all claims, damages, and causes of action, of any nature or kind whatsoever arising from the collection, use, retention and disclosure of all the personal information provided
